- All owners must be present with a current PA Driver's License or PA ID
- Vehicle information (VIN number, Plat number or Title Number)
- A Vehicle in a business name will need an authorization letter on company letterhead signed by the Owner/Officer of the company.

- The new specialty plate

- An MV-145A filled out by a Health Care Provider or Police Officer
